This page lists all the hiring paperwork required for casual hires. Please download, complete, and return all of the following documents to Eric Meyer at em3255@columbia.edu as soon as possible.
Schedule your I-9 in-person appointment immediately.
Please note your name and address must match across all forms and your name must match your passport or ID.
Documents Checklist

- Casual Employment Form
- I95 NYS DOL Form
- W4. Please password-protect any documents with your SSN for your security.
- IT 2104. (If you are currently in New York State, fill out IT 2104 and use your current address on all forms) or non-Resident IT 2104.1. (If you are outside New York State).
- Completed I-9. This is a two-part process. Please complete Section 1 online and then make an appointment to complete Section 2 in person at Kent Hall (required). If an appointment is not available soon at Kent Hall, walk-in appointment hours are available at Studebaker (see below). Please let us know the date that you have completed your I-9, your appointment cannot be processed without it. Students who already have a valid I-9 do not need a new one but will need to provide the date it was completed.

- Information Form
- If you are not a US-resident also send a copy of your visa, 1st page of your passport, I94 and one of the following:
- I20 form for F1 visas
- DS2019 form for J1 visas
- 797 form for H1 visas
